Small power switch power supply, surge protection IC exploded—what could be the cause?

You need surge protection for your power input; connecting it directly to the chip will definitely cause it to blow.

Use components such as varistors, TVS diodes, and common-mode chokes. Depending on the protection level required, consider adding gas discharge tubes as appropriate.

You can also add a pi-filter (LC filter) at the subsequent stage. The required protection level determines how robust your protection circuit should be. You can solder the circuit and gradually increase protection levels to observe effectiveness.
